Overview:
The AMBAR (Alzheimer Management By Albumin Replacement) study is a phase 2b/3 randomized, controlled clinical trial that evaluated the effects of plasma exchange (PE) with albumin replacement in patients with mild-to-moderate Alzheimer’s disease (AD). The study was published in 2020 and involved 347 patients randomized into four groups: three PE treatment arms with different doses of albumin and intravenous immunoglobulin (IVIG) replacement, and a placebo (sham PE) group.
The treatment groups underwent a 6-week period of weekly conventional PE followed by a 12-month period of monthly low-volume PE. The study found that PE-treated patients performed significantly better than the placebo group for the co-primary endpoints, showing less decline in Alzheimer’s Disease Cooperative Study–Activities of Daily Living (ADCS-ADL; P = .03; 52% less decline) and a trend for less decline in Alzheimer’s Disease Assessment Scale–Cognitive Subscale (ADAS-Cog; P = .06; 66% less decline) scores at month 14.
The AMBAR trial also showed that the high-albumin + IVIG treatment group exhibited the greatest metabolic stability over the course of the study, indicating the smallest percent decline in metabolism and the least progression of defect compared to the placebo group. This suggests that PE with albumin replacement may help slow cognitive and functional decline in AD patients.
